Federal and state regulations (such as HIPAA) protect patient information, so it is important that proper authorization is obtained. Also please note that, in accordance with Tennessee state law, there may be a charge associated with obtaining a copy of your records and photo identification is required.

For your convenience, a copy of our HIPAA compliant Release of Information document is available in our medical records department. Joint Class Registration

Registering for Joint Replacement Surgery Seminar
  • This will help us contact you for additional information about the class if needed, and to confirm your registration in a seminar.
  • Classes are held every Monday (with the exception of Memorial Day and Labor Day) from 10 a.m. - 11:30 a.m. at Parkwest Medical Center. We will try to honor your first request of class date, but if that date is already full we will suggest the next available class.
